Seminary Registration Form

The Seminary Registration Agreement is a legal requirement. Parents are required to consent to it only once during the time their child or children are in seminary.

When parents or guardians sign or agree to the consent, they legally agree to allow Seminaries and Institutes of Religion (S&I) to track their child’s or children’s seminary progress using WISE.

Registration:  A student must be 14 years of age and in year 10 OR year 10 & turning 14 during the year.
